1043 Giant Spindle Cell Lipoma of the Perianal Region: A Rare Cause of a Perianal Mass

Abstract Background The anal and perianal region is an anatomically complex region. differential diagnosis for masses broad can represent both benign malignant lesions. Case Presentation We present the case of a 63-year-old gentleman who presented to outpatient clinic with large mass which had been one year. was increasing in size becoming painful. He did not report any alterations bowel habit, per rectal bleeding or discharge. His past medical history significant type 2 diabetes mellitus hypertension. reported 40-pack year smoking history. On examination, there pedunculated soft tissue arising from left side area measuring approximately 12cm diameter. MRI scan demonstrated appearing polypoid lesion originating buttock close but separate internal sphincter. underwent elective examination under anaesthesia (EUA) excision lesion. multilobulate involving external sphincter, it excised en bloc via elliptical incision. Histological assessment revealed spindle cell lipoma. patient uneventful post-operative recovery given histology findings no further adjuvant treatment considered necessary. Discussion Spindle lipomas are rare, lesions characterized by classic triad mature adipocytes, ropey collagen, uniform spindled cells. Surgical represents first line treatment. Conclusion Perianal often pose diagnostic challenges this highlights importance pathological assessment.
